We realize that the amount of paperwork is cumbersome. However, it is important that we have as much information as needed to give excellent care and remain compliant with federal, state and insurance regulations.
All patients should bring a photo ID, insurance cards and a current list of medications.
New patients who have printed and completed new patient forms should arrive at least 20 minutes prior to appointment time. If you can’t print the forms to bring with you, we will have them ready when you arrive. Please come 30 minutes early to complete the forms in the waiting room.
Returning patients who have not been to our office within the last 6 months, should print and complete the returning patient packet. If you can’t print the forms to bring, come 30 minutes early so that you can complete them before your appointment time.
We understand that some questions are repetitious, but this paperwork is required and appreciated.
